Cedar City would like to know how you feel about Cedar City’s trails, parks, Aquatic Center, sports fields, recreation programs and other facilities. The survey is absolutely confidential and the findings will be used to help us plan future venues and programs for citizens and visitors.
To more accurately gauge family participation and interests, this survey is intended to be completed by the head of household on behalf of all those residing in his/her domicile (house, apartment, etc.).
Please note: Only one survey per household is allowed.
